The Part Of Addiction No One Is Talking About

As a clinician who works largely with addictions and the intersection of trauma and life, I get to be on the front-lines in a seemingly unending parade of adventure and experience. I picked up a heroin/fentanyl rig from our bathroom this afternoon. I walked around in the blue gloves offering prostate exams but for some reason there were no takers. Yesterday I spent the afternoon with my partner in crime, Dawn Taylor, consulting on one of the most complex cases you could imagine. That day began with an RCMP corporal at the homeless shelter, someone had procured a video projector, probably from the same inexhaustible storehouse where all those nice bikes come from.

Addiction is a complex and fascinating issue, if you can somehow remove the never-ending cacophony of tragedy and loss. It has taken me years of full-time employment to come to understand just a small part of this complicated thing, even with it so present in my own history. The information we all read in the media, in what we assume are serious articles written by addiction experts, is ofttimes laughable. This week I read again, for the 400th time, about the debate whether or not marijuana is addictive. I dare you to say it is, or it’s not, or have no opinion at all – it won’t matter, you’ll still be called an idiot. The misinformation and self-delusion from both sides of the McInformation Universe make it next to impossible for the average Normie to retain any level of informed dialogue. Is marijuana addictive?

So much of what passes for real information is the reality television version of reality. These same websites and professionals are often completely unfamiliar with the subculture they are lecturing. Just because you have a Ph.D. and get to charge thousands of dollars because you are the flavour of the month doesn’t necessarily guarantee you know what the hell you are talking about.

Being a stoner is to be a part of a community, of sorts. This subculture brings it’s own rules and mores and values which are impossible to truly understand from the outside, much like the priest who is giving marriage advice. I am not saying that addictions counsellors who have not been druggies have a lesser understanding and therefore are less effective. So much of what we do in the counselling room has little or nothing to do with your drug of choice. Impulse control, trauma, and mental health are all universal issues.

It doesn’t take an addict to help an addict but it does take one to understand how funny it is that we managed to convince you that the only reason we wanted medicinal marijuana was for the physical pain. I studied the outlet across the street from my addictions office for 6 months and you would be impressed by the number of Ford F-150’s that represent at 4:05 pm on a weekday, driven by two dudes straight off the construction site who jog across the street to the store. The last time I was in there the owner offered me a “pull” from her vape, but I’m guessing that was for the pain that she inherently recognized in my burdened soul because weed gives you wings. You can send me angry emails until the cows come home but it won’t change the fact that my job entails knowing how to separate the truth from bullshit. I have clients who use medical forms of cannabis and understand the difference between CBD and THC and aren’t really interested in being baked all day; and I’ve seen dozens of clients who show me their Medical Marijuana Card and we have a good laugh. It’s all going to be a mute point on October 17, except that now a whole bunch of us have decided to try it out, now that it’s legal. I polled several rooms in the past couple of months and that’s a real thing.

Don’t get me wrong. I barely care anymore, it’s just a reality in my world. This article is not about the evils or benefits of weed, so don’t go off on me. Dawn Taylor and I have an entire upcoming podcast on pot coming this winter that is guarenteed to upset a few of you, so hold on. (Yes we are launching a podcast this winter called “Reality Therapy” with the tagline “Welcome To The Argument”).

I set out to write an article about how difficult it is to leave the life, even when you want to quit and it’s clearly time to move on. As usual, I meandered a little. All this diatribe was leading to the realization that quitting the self-medication game is a lot harder than most people think, and all too often that addictive personality thing we’ve all heard about is more a learned behaviour than a genetic one. Your dad may have been an alcoholic, and that may be why you think you want a drink right now, but it’s also possible that when you were a preschooler you watched your father self-medicate his tragic life and that taught you a little something. Perhaps your parent was addicted to rageahol, and you figured out that it’s way easier to check out than hang in. There are literally hundreds, nay thousands of ways we have learned to cope with childhood trauma or a controlling parent or that uncle who liked to touch you. You were the fat kid, or your complexion was a mess, or you were too shy to be the popular kid in school.

Name your poison.

Years of continuous drug use will create neural pathways hardwired to thirst for “altered”. Most drug users have difficulty imagining a world where they could not be buzzed, the real world can be incredibly boring or painful or confusing. I’ve watched hundreds of clients wrestle with the reality that they will never really get that monkey off their backs because they’ve been doing this for so long that they’re probably going to have that little primate in the back of their mind for the rest of their life.

caffeineI haven’t used cocaine in 25 years but that doesn’t mean I would sit in a locked room with a line of coke, if I knew no one would ever find out. Besides that, I may not jones for blow, but there is always something vying for my attention, and not always something good. People self-medicate with booze or shoes, chocolate or masturbation or video games – the delivery system seems less and less important the longer I do this. I could tell you stories of people who’s lips were raw because they ate 4 bags of sunflower seeds a day in an effort to placate the persistent hunger for meth or crack or because their anxiety is raging and it’s the closest thing at hand. People are clean for 15 years than they “go back out”, god only knows why. It might have something to do with the fact that  you quit your drug of choice but never moved beyond that need to feel good even if it might feel bad after you come down from that sugar rush. I don’t usually tell clients that happy little factoid on our first visit together.

And let’s be honest, it’s not just the “junkies” we’re talking about. You went to that doctor after you hurt yourself and 6 weeks later you had a very hard time when your prescription for Percocet ran out. Addiction is easy because drugs really are as good as people think they are… for a while. I’m sorry but your youth group has a hard time competing with the spiritual orgasm that Dexedrine can deliver. If I’m completely honest with myself I’d love to get buzzed, some days.

Addicts have the best of intentions when they quit using. You might be surprised at how honest and kind and sincere a person can become when they are in The Program. Many sufferers are able to clench down and sweat it out, but that whole piece about being a normie for the rest of my life was harder to accept on an emotional level than I could have ever imagined. The rest of your life completely sober doesn’t sound as sexy as being blitzed on shrooms with good friends this Friday night. The relentless normal is difficult for some people to handle, week after week, and it isn’t long before recovering addicts often feel the claws come out from their backpack of chimps (I know, chimps are not monkeys; I ran out of snappy monkey references).

I have been told, more times than I can remember, how much harder it was to leave the life than the drug. As the sarcastic quip goes, quitting is easy, people often do it several times a week. Staying sober when all that stuff you ignored when you were high comes back calling, and you have no way to self-medicate because that group said you had to be completely clean, now that’s not so easy. This sounds allot like the ending of Goodfellas but I can’t help it, it’s a powerful and persuasive subculture. As Henry Hill said in that movie after he was placed in Protective Custody:

“See, the hardest thing for me was leaving the life. I still love the life. And we were treated like movie stars with muscle. We had it all, just for the asking. Our wives, mothers, kids, everybody rode along. I had paper bags filled with jewelry stashed in the kitchen. I had a sugar bowl full of coke next to the bed…Anything I wanted was a phone call away. Free cars. The keys to a dozen hideout flats all over the city. I’d bet twenty, thirty grand over a weekend and then I’d either blow the winnings in a week or go to the sharks to pay back the bookies. Didn’t matter. It didn’t mean anything. When I was broke I would go out and rob some more. We ran everything. We paid off cops. We paid off lawyers. We paid off judges. Everybody had their hands out. Everything was for the taking. And now it’s all over. And that’s the hardest part. Today, everything is different. There’s no action. I have to wait around like everyone else. Can’t even get decent food. Right after I got here I ordered some spaghetti with marinara sauce and I got egg noodles and ketchup. I’m an average nobody. I get to live the rest of my life like a schnook.”

Leaving the party to become a citizen often feels like it is going to suck. All you have to do is change your everything. It’s way easier to just give up and slowly succumb, in spite of that false narrative you keep telling yourself, the one where you don’t end up in my office.

Injustice And The Third Way

It isn’t fair and I don’t like that. On some level, most of us have had to face the harsh reality that doesn’t feel fair. But is it?

No. There is no Pixar ending.

Sitting in the old counselling office one rainy day this truth began to open up wider for me. I began to see threads, real or imagined, in many of the stories of pain and trauma that funnelled their way through my door and sometimes my life. It may be that this cognitive distortion, this need for life to “make sense”, has been responsible for a larger slice of misery than at least I ever imagined.

The stories are similar. Depression or anxiety brought on by trauma or heartbreak with a sense of cruel incongruity. We have been wronged and something needs to be put right. We simply cannot accept that there is no payback. It’s so… unfair.

As the good book says, and I proceed to steal and compare myself, albeit tongue-in-cheek, to the great Apostle Paul, “I do not come to you as one who has attained. Nonetheless I press on.” Like you I wonder why good people seem to suffer and total jerks continue to prosper. She left and it hurt and she never came back. I know that feeling. It’s bad. He died without ever getting his “just desserts”. Some of those Nazis escaped to Argentina and lived off the rewards of their raping and pillaging and genocide. As I consumed Martin Davidson’s book, The Perfect Nazi: Uncovering My Grandfather’s Secret Past, it was apparent that the author never was able to see his grandfather pay for his heinous crimes. There is no glass slipper ending. Gru doesn’t really adopt the cute little girls – they get thrown into the social welfare system and spend their childhood in Foster Care. They became sexually active early. Chances are they have issues with addiction and end up marrying poorly. That’s the real world and it is many things, but it is not fair.

But here’s the problem. I am DNA’d to believe in fairness. I cannot seem to get it through my dense skull that reality cares very little about my sense of injustice. It is up to me alone to move forward, and it’s very, very, hard. Years.

There are three ways, I think. There is the way I want, the way I don’t want… and the way I don’t know yet.

I have a friend, a real friend who I see in the real world, who has been working on her stuff for years. Not too long ago she came to me and uttered, “Five years. It’s been five years and I’m still not fixed.” It was heartbreaking, because she is a rock star.

As I wrote in my last post, some people go through things that are beyond coping. There isn’t a toolbox in the world that prepares you for the loss of a child. As I’ve said even recently, you get a free pass for that. I would go crazy. I cannot imagine a scenario in which I would be ok if I lost one of my kids. It’s simply not reasonable to imagine, yet in ways that are beyond my meagre understanding, some people keep going.

I just finished rereading Gladwell’s David and Goliath: Underdogs, Misfits, And The Art Of Battling Giants. He’s a storyteller. I like stories. In that book he points again and again to average people who, when faced with insurmountable odds, found a third way. He doesn’t call it that, but he seems to understand. Mennonites who have learned that vengeance sometimes creates more pain than it heals, even when your baby is defiled. Huguenots who stood up to the scariest dictatorship we have known and said, “We have Jews, come and get them”. Poor people and zealots and losers who came to understand that they have only one life; and the need for justice was ruining what little they had left.

There is the way I want, the way I don’t want… and the way I don’t know yet.

Five years of reading, learning, letting go, and moving forward against impossible odds. No one comes to a counsellor to sign up for that program. Most of us who are in process would happily abandon it at any time for a magic pill or a glass of good tequila.

Like most of us, it started with a broken toolbox of coping mechanisms. Imprints from childhood and generations of dysfunction, broken promises and unfulfilled expectations. Death had robbed her of a piece of her life. Choices. No role models who were not also broken. That ugliness inside. So much pain and hurt it was overwhelming. Some of you have been there… are there.

It’s hard to believe that you might be a success story. There is no pink cloud, no happy ending, no days of bliss without Xanax. But on some deep levels, people can fundamentally change. She still reads everyday because, if nothing else, she has learned that there are no shortcuts to wisdom. She needed to get started. There were days and months and years when it didn’t seem that there would ever be a good day again, but she was wrong. She knows that now. I don’t talk to her like a counsellor anymore, we just exchange information, sometimes everyday. There has to be a good reason to work this hard or I couldn’t live with myself and do what I do for a living. I’ve seen too many people find a third way to believe that my life is hopeless anymore. There must be a way from where you are to where you need to be or I quit.

Here’s the last thing I’ll say about this here. I didn’t have a clue what the third way was for years. I thought, many times, that I had this covered, but I was very wrong. I knew what I knew and I was willing to die for the cause, but the cause was flawed. No one could tell me that because I was right.

But one day I realized I wasn’t. Letting go of what I knew to be fact was exceedingly difficult.

Realizing that I had to work on this stuff every day for the rest of my life was at first disheartening, then exciting. It sure didn’t start out “exciting”. It was horrible. How do you stop believing what you have known to be true all your life? How do you “fake it until you make it” when you feel like you are lying to yourself? How do you hope again when you have been proven wrong in hundreds of ways?

Those are very good questions and this isn’t Sunday School and we have to figure this out for ourselves (with a little help). I can’t give you a slick closing sentence to make you or I feel any better. As I have often been counselled, I need to be relentless in my desire for change. There is only one game in town.

Rewiring Your Brain

A new study reported by the Huffington Post, among others, is reporting that cocaine begins rewiring the brain even after a single usage. This is old news to those of us who deal with addictions or have ever taken a class on neurochemistry. Drug and Alcohol Counselors have known for years that the opiates, though seemingly innocuous when taken at prescription strength (T3’s, Percocets, Emtec, Morphine, Oxys, Heroine, etc) have a profound and physical effect on a neurological level. Unfortunately for many of us, so does porn. Actually on some level any response mechanism, coping techniques, cognitive distortion or belief has not only a physical but also a neurochemical effect on your brain. There are fantastic and crackpot websites a plenty to explain this all over the internet. Some are informative, some are… less informative.

It’s important to understand that the brain is not a static device, set in stone as they say. It is actually possible to change the way the brain spits out those little chemicals and where those little dudes land. If you don’t believe me just start or stop a habit. Creating a habit is nothing more, on a chemical level, than rewiring where your dudes land. You can change the way you act, the way you think, what you believe, who you are. This is powerful information if you know how to manipulate it. You are not a victim of your circumstances, at least on a neurological level. You can convince yourself of virtually anything, given enough time and effort. It’s a fascinating study that has pragmatic consequences. If you don’t believe me google neuroplasticity, or synapses, or dendrites, axons or neurons and you’ll soon have a ton of new material to throw around at parties to impress your friends.

Psychology has come a long way since we liked to  drill people’s heads, and information is power. Once you realize that quitting smoking, or stopping catastophizing, dealing with your poor self-esteem, or stopping using cocaine is a matter of rewiring your brain it is possible to hope that change can come.

You can do it. It’s a scientific fact.

Reflections From The Road (Part 2)

Winfrey on the first national broadcast of The...Turn on the TV anytime and you can find amateurs and professionals with varying degrees of emotional snake oil sales gimmicks for helping you deal with the pressures in your life. Oprah has made billions pretending to be a psychologist and has launched the careers of myriads of self-help gurus. Arguably she has been an excellent proponent of self-care and has called attention to the plight of those struggling with mental health issues. I owe her a debt, I suppose. She and her bald friend, among others, have made going to a counselor fashionable. It is no longer taboo to see someone about your failing marriage or your personal problems. I fear, however, that the pendulum may swing too far now that obesity and anxiety, as well as depression and addictions, are now reasons to go on disability. While this may help some, many others will undoubtedly line up to exploit the opportunity to get ‘something for nothing’. Even more problematic is the reality that going on disability is often the worst thing some of these people can do. Every day at the addictions center where I work part-time I have to bite my tongue when another addict goes on disability in spite of the fact that their ailment is self-induced and some are looking, yet again, for a feel good solution to a difficult problem. Not working can be the absolute worst thing you can do in addiction recovery. We have known for generations that “idle hands are the devil’s workshop”.

Recently I read a great article  on Cracked.com  called, “You hate yourself because you don’t do anything”. It’s one thing to take time to address your problems. It’s another thing altogether when a recovering addict hasn’t worked in years and blanches when I suggest one of their problems may, just maybe, be that they have way too much time on their hands.

In counseling I often come across unemployed people who tell me they are too busy to do what I am suggesting they need to do to get better. I know this is politically incorrect but I often look them straight in the eye, screw up my most professional looking expression, and ask, “Are you unemployed busy or really busy?” Granted some people are doing many good things, but often those I run across will admit that they had to come to see me at ten and have to go to the Walk-In Clinic at three – they’re swamped. They have succumbed to a lifestyle that lacks productive routine and are not working towards re-engaging in society. We only have one life, one precious and finite life, to experience and contribute. Spending that life in poverty and boredom, when we had other options, is a tragedy that affects the generations to come. Don’t misunderstand me, I am a huge proponent for the plight of the poor and hurting. That is why I am so passionate when such a lifestyle is avoidable. Some people need to get off their ass and help themselves. No one cares about your problems as much as you do and this isn’t television – there is no knight in shining armor, no billionaire talk show host that is going to swoop down to rescue any of us.

I work in a Fibromyalgia/Pain Clinic and see hundreds of people every year who have been dealt the worst hands imaginable. I have patients who can barely move, let alone work. Every week I speak with the most amazing people who feel they have “lost their life” to terrible illnesses. Their courage to move forward, in spite of chronic pain, fatigue, and illness, is inspiring. Many have been abused by the medical system and offered horrible medical and psychological care. They would love to work again, run again, contribute again, but cannot. It is maddening how few of them are accepted for disability, and they are often subjected to the cruelest interrogation by insurance companies who belittle them and subject them to emotional and financial hardship. There is something wrong with a system that gives money to able-bodied twenty year-olds who are too lazy to work yet rejects someone who has legitimate and life-altering illness.

Personal growth is about movement. I am writing a book right now called, “How to Improve Your Life 52%”. Real change is about doing the little things. As I have said on many occasions, drastic change is rarely lasting. It is all about momentum, moving forward inch by inch, a little bit day-to-day. Maybe that means getting up at the same time every day. Perhaps that means listening to an audiobook on psychology or self-help. Some of us need to start down the road to reconciliation by making a phone call. You might need to put down that doughnut or only smoke ten cigarettes a day. The key is to get started – do something. Doing something little is always better than planning to do something big. Sometimes change starts by getting off the couch. You may not be able to do a hundred pushups but you can do one. Baby steps.

I think I will take my own advice and get up and stop writing…

What Do You Want?

I Can't Quit You BabyPeople often come to counseling hoping that the professional will basically condone what they have already been doing to deal with their problems. Eventually that counselor, if they don’t suck, will gently point out that perhaps, just maybe, the problem isn’t everyone in their screwed up family – the problem is how they are handling their thinking, coping, and life. This is usually a difficult thing to hear and process. Such a revelation may necessitate change in areas the client is not happy to address. They want to be different but they “cannot” change what they need to change. At some point they will turn to their counselor and actually ask for help doing “something they don’t want to do”.

I won’t teach you how to quit doing something you don’t want to stop doing. I have a hard enough time convincing patients to spoil themselves. Besides, people usually do what they want to do. So the question is, what do you want to do?

Here’s the secret – don’t change what you do, change what you want. How easy would it be to quit drinking if you earnestly believed that you hated alcohol and didn’t want it in your life anymore? The key isn’t to convince you to stop snorting cocaine. The key is to help you learn a different way to think about cocaine. A different perspective will change everything.

I have a client who wanted to stop using cocaine so one day he lined up a line of cocaine and then made a second line out of Drano, a horrible cleaner that was under the sink. The two lines looked almost identical and he asked himself, “Which line is worse for me to snort?”

The answer seemed obvious, the cocaine was obviously safer to snort than the toxic drain cleaner. This is the obvious answer and the obvious answer is completely wrong. Snorting the Drano will cause him to become sick and throw up. The experience will teach him never to have that experience ever again. Problem solved. Snorting the cocaine will lead to something that feels good but will take your house and your marriage. It is much much safer to snort the Drano.

You don’t need to do something that you do not want to do. You simply change the way you feel about the cocaine. You consider soberly how prone you are to remember only the good parts of a bad addiction. You allow yourself to believe that you could be happy without artificial stimulants. You begin to dream about life in Normieland. You start getting up in the morning. You get a job. You go to church, or yoga, or NA. You choose to stop entertaining your negative thoughts and force yourself to be positive until you believe it. You come back to life.

The principle applies for almost everything we are dealing with. Radically changing the way we think about life is the ONLY way to find wholeness as we learn to address our inaccurate thinking patterns, our dysfunctional coping skills, and our skewed outlook on life.

As we say around here all the time, “Change your mind and your butt will follow”.

I have radically changed the way I think about addictions.

I work part-time in addictions and see it’s effects literally dozens of times each week. It’s easy to believe that the problem is the addiction – if we can just help people stop drinking than their life will work itself out. Unfortunately this is not even remotely true and people who understand people are realizing that the addiction is simply another symptom of something much deeper.

When I was young and drugs came calling they were just another solution to the problem called “My Life”. Chocolate made me happy right now. So did cocaine and boobies and volleyball. Basketball sorted me out, so did pot. My only crime was that I grabbed too hard at one of my solutions to stress. Why couldn’t I have developed an addiction to body-building instead? Chocolate is nice, why couldn’t it have been to chocolate?

Dealing with your maladjusted life by stopping only one of the symptoms does not make sense. Somewhere along the line in many lives drugs became medication, not recreation. Cocaine helped you not have to think about your crumbling life. Drinking and sex helped you believe you were important. Being high kept you from thinking about your struggle to hope that things could change.

In counseling I encourage clients to look beyond their need for medication and address the actual disease they have been medicating. We need to learn to put our lives in perspective and change dysfunctional thinking patterns. Taking responsibility for your own heart and happiness truly is the best thing you can do to improve your life.

 

Does It Really Matter What You’re Addicted To?

It’s all about dopamine… and Chapstick.

The need to flee “ordinary” motivates many of us to indulge in the things that others will someday call our “addiction”. Professionals, including myself, have long delighted in shutting down those who believe they had an “addictive personality”. But what if there is more to this than I first supposed?

People working in the addictions field can give you copious examples of clients who were “poly” drug users, addicted to whatever was available. These same addicts would, while in recovery, be the first to sell out to the program, find Jesus, and plan to become an addictions counselor. People who are impulsive, struggle with impulse control, and prone to show a willingness to try anything for a good time are prime candidates for poly-drug use and “all-or-nothing” thinking. Whether it’s Chapstick or heroin, sunflower seeds or cocaine, exercise or meth, addictions all serve the same basic primary function – distraction. As I have heard countless addicts say, when asked what drug they are addicted to, “What you got?”.

Studies have shown that dopamine levels begin to rise long before someone actually snorts cocaine, for example. Just the thought of getting high on Friday is enough to alter the chemicals in our body today. This release of happy goodness serves to focus our energy on satiating our addiction while distracting us from looking at the situation more objectively. The mental build-up to our addiction warps our perception of reality and gives us tunnel-vision. This tunnel-vision is why addicts consistently choose their drug over their family. They truly believe that their family is their top priority but cannot, once the thought has become an intention, stop themselves from making a bad choice over and over again.

I have known people who get this same euphoric energy and satiation from shopping for shoes, or going to garage sales, or running on a treadmill. Addictions experts recognized this before the mainstream medical community and began recommending addictions deflection – moving unhealthy vices into less and less harmful activities through transference, not a “cold turkey” approach.

‘Life’ is usually the reason most people have problems with addiction and impulse control. Helping someone stop drinking, for example, is only a very small part of staying healthy. Learning to deal with their dysfunctional coping skills that have helped them survive their horrible lives, now that’s the real crux of the matter. The journey is not hopeless but it is fraught with work and frustration.

I know what it is like to feel bogged down by the pain of life and struggle to even care if things got better. I do not profess to understand what you are going through and have grown too tired to give you three simple steps to fixing your life.

All I can say is that it was worth it.

In spite of firmly believing that my life would never get better somehow I allowed myself to admit that I was afraid of things getting better. I forced myself to hope again.
